| 1 | Friends | Across Series 3 - 5, Ross remains the same age (29) for all three years. In Series 3 he tells Chandler, "No thanks, I'm 29" when offered chocolate milk; in Series 4 when the guys try to party without Gandalf they all say they're 29; and in Series 5 when he's trying to reconcile with Emily, he says either he does, or he would get divorced for the second time before age 30. |
| 2 | The Simpsons | This is a mistake for the introduction for the later episodes. When Homer screams, he turns round. You see this in a wide shot. There are no boxes to the right of the door in the garage. However, two just appear out of thin air when he runs through the garage. |
| 3 | Full House | In the episodes where Michelle is a toddler, during the opening you can see her point to the camera during the car/bridge part. Jesse tries to make it look cute by laughing and pushing her arm down. |
| 4 | Sex and the City | At the beginning of every show we see Carrie walking down the street and she gets splashed by the bus. First the bus has people on it, then in the next shot they all disappear. |
| 5 | Lost | In numerous shots of individual characters, while on the island, the sweat stains on their clothing significantly and repeatedly change, or disappear and reappear between shots. |

| 6 | Charmed | When the Charmed Ones' father, Victor, came to San Francisco in episode 1-3: "Thank You For Not Morphing," his last name was Halliwell. But when he came back in the third season and later, his last name was Bennett. |
| 7 | SpongeBob SquarePants | Ep 3-21 Spongebob Meets The Strangler / Pranks A Lot: When the invisible Spongebob and Patrick scare Sandy, they are using a sheet to appear as "ghosts". However, the form of the sheet reveals that they don't have their helmet bowls on. How could they survive in the treedome that time? They sounded fine, not dehydrated. |
| 8 | Malcolm in the Middle | When Dewey yells for Lois because the bow was put on the present, if you look to the left of Lois, you will see a crewmember kneeling down, handing Lois the red dress, who is holding a bucket of water. He quickly gets out of the camera's view. <em>[This is on the DVD of the first season. Someone said on the audio commentary that after catching it, they deliberately left it in to see if anyone would spot it. It's still a mistake, but that's why it's there.]</em> |
| 9 | Mighty Morphin' Power Rangers | When the Villains send their cronies (for example, Putties, Tengu Warriors, Cogs, Piranhatrons, etc.), their numbers constantly change from when they arrive, to when they leave. |
| 10 | Prison Break | Season 2, episode 9. After Lincoln and LJ have been recognized by the waitress, they speed off down a road. During their conversation in the car, whenever we see LJ from (his) right side, his hair is blowing in a strong draft, and is blown away from the temple. In all other shots, his hair is completely calm, and is hanging down and forward, covering his right temple completely. |
| 11 | Buffy The Vampire Slayer | In the episode 'I Only Have Eyes For You', during the flashbacks to 1955 James and Grace dance, and James later shoots himself, to the song 'I Only Have Eyes For You', which wasn't released until 1959. |
| 12 | 24 | When Nina's just been shot, and makes it to the barn to call CTU (around 7.10am), while she's speaking on the phone there's a cameraman visible on the right hand side (DVD). |
| 13 | Days of Our Lives | When Shawn is talking to Belle over the phone, the Eiffel Tower can be seen outside her window, yet, she's supposed to be in Milan, Italy. |
| 14 | Scrubs | Season 6 - Episode 1 - My Mirror Image: Dr. Cox's hair colour and length change back and forth significantly during the episode. |
| 15 | Dark Shadows (1991) | When Barnabas enters Josette's room, the door is painted white. When he leaves and Natalie follows, the door has changed to varnished wood. |

| 16 | Alias | In the episode where Sydney is supposed to be in Helsinki, there are visible skyscrapers in the background. There are no skyscrapers in Helsinki. |
| 17 | Family Guy | Brian's newspaper's name changes from "Daily Informant" to "Daily Tim...". The newspaper also turns upside down between the two shots. |
| 18 | Eastenders | When Cindy wanted a hitman to kill Ian she looks in the yellow pages to find one. Surely the police and the makers of the yellow pages would of been alerted if a hitman agency wants to advertise itself in the yellow pages? |
| 19 | The Fresh Prince of Bel-Air | When Will staples his Malcom X poster to the wall, the wall bends back and forth for a second or two. |
| 20 | South Park | In the classroom there is a poster of Christopher Columbus in the back wall in the class. In the last shot of that scene you can notice that the poster has disappeared and is replaced with an astronaut poster. |
| 21 | Red Dwarf | In the first episode, 'The End' when Lister is telling his door to lock so he can get his cat out you can see the hand of a member of the crew pushing the door shut (look out the window). |
| 22 | Frasier | There was an episode in which Martin attempts to end a feud between Frasier and Niles by telling a story about his detective partner. He starts with the line, "I never had a brother...", but in the episode about Frasier's cousin Nikos getting married, Martin's brother Walt was introduced. |
| 23 | Smallville | Clark's hands on the hood of Jeremy's truck shift in different shots from spread wide to in front of him and close together. |
| 24 | The Fairly OddParents | When Timmy is turned into a girl by a supposed wish, shouldn't he/she? still wear the pink hat? (Note: In "Secret Origin..." we know the pink hat was a girl hat.) |
| 25 | Mr. Bean | In the swimming pool scene, we see the female lifeguard swapping places with the male lifeguard on a raised seat by the pool. However, shortly afterwards, he tells everyone to leave the pool and is not on the platform. (Why would he go up to the platform in the first place if everyone was getting out shortly anyway?) |
| 26 | House, M.D. | As House describes the lady's symptoms, in the front shot Chase has a small book/pamphlet in his hand. The camera changes to a side shot in the middle of House's description and suddenly the book is on the table and no longer in Chase's hand. This happens right in the middle of House's line and Chase doesn't make any quick movements to account for this discrepancy. |
| 27 | Alias Smith and Jones | An egotistical, prissy and dishonest lawyer like Mr. Fletcher wouldn't make the mistake of allowing his name to be misspelled on his office door. It reads "Winfred Fletcher." His first name, as it's pronounced throughout the episode, is "Winford." |
| 28 | Star Trek: The Next Generation | In the episode "Reunification part 2" when Picard, Spock, and Data have just escaped from the female Romulan commander, when they are walking out of the room look at the green glass ornament - you can see reflection of a crew member. |
| 29 | Boy Meets World | When the house is shown from the front exterior, there is a window on the left, but when seen from the interior there is a hat rack there. |
| 30 | The Prisoner | The Villager Rover attacks changes outfits several times between shots. He's wearing a striped sweater that turns into a pink jacket that turns back into the sweater, etc. |
