Continuity mistake: In the opening, the camera zooms in on an eye that is a very light greyish blue. The camera then cuts to a man lying on the floor. The implication is that it's this man's eye. However, the man's eyes are brown.

Continuity mistake: Right at the end where we see Sarah still in the cave with her daughter and her birthday cake, the number of candles on the cake changes from 5 to 6 between shots.

Continuity mistake: When Buffy goes to save Xander from Lissa, she knocks a dagger out of her hand and it is thrown to the ground near the far end of the room. Two shots later it has moved closer to the door.

Continuity mistake: When Dewey and Gilleece are attempting to jump aboard the runaway train by driving along side its front cab, the passenger side door is ripped off by crashing into a train signal.
